Position Description: The Community Health Navigator is assigned to the Local Capacity Building (LCB) Section of the STD Control Branch. Under the direction of the Local Capacity Building Section Chief, the Community Health Navigator (Behavioral Health Counselor III) is a trained health professional who provides case management and health education counseling to pregnant clients diagnosed with syphilis, including following women after treatment through to delivery to ensure infants are appropriately evaluated at the time of birth. This position is responsible for assessing clients, determining needs, and actively linking high risk pregnant women with syphilis to prenatal care, substance abuse treatment, behavioral health services, and other services as indicated. The position will conduct field visits to locate, interview, and refer pregnant women with syphilis and their partners/contacts for follow-up as indicated; serve as the link between public health and private health care providers; confirm diagnosis, testing and treatment; and provide patient education and counseling regarding risk reduction and facilitate partner services.

The Community Health Navigator position is also responsible for (1) supporting the implementation of local Congenital Syphilis Morbidity and Mortality Review Boards (CS M&Ms); (2) conducting targeted provider detailing on congenital syphilis prevention; and (3) establishing the Fresno County Syphilis Task Force. This position will engage with key local partners in CS prevention including maternal, child, and adolescent health (MCAH), substance abuse treatment programs, behavioral health, correctional health services, local harm reduction programs, and family planning service providers, among others. This position will leverage and build upon existing county relationships with Medi-Cal Managed Care Plans (MMCs), Federally Qualified Health Centers (FQHCs), and Preterm Birth Initiative (PTBi) in Fresno County to work on effective linkages to services.

The position requires a high volume of travel (90%) within Fresno County and the surrounding Central Valley region to conduct case management activities, field work, project specific meetings, and provider visits. This position ensures the protection of individual confidentiality at all times and maintains confidentiality of medical and/or epidemiological information about patients.

Please Note: The funding for this full time contract position ends on December 31, 2018. The contract may renew and is contingent on renewed funding.

Qualifications: Required Qualifications: • Bachelor's degree in related area and 3-5 years relevant experience and / or equivalent experience / training. • Valid CA driver license and proof of valid insurance coverage. • Thorough knowledge and understanding of case management principles, methods, procedures, and standards. • Available for frequent travel around the Fresno and Central Valley area, with occasional travel to other parts of the state. • Proficiency using Microsoft Office 2010, including Outlook, Word, Excel, and PowerPoint. • Ability to develop and present educational programs and / or workshops. • Highly effective problem-solving and critical-thinking skills. • Ability to work in a highly collaborative manner with a diverse client group. • Highly effective verbal and written communication and interpersonal skills. • Ability to collaborate with a multi- disciplinary group of health professionals. • Ability to work independently and work well under pressure. • Ability to manage multiple priorities and effectively meet deliverables for multiple projects. • Ability to reason and problem-solve analytically, logically, and creatively. • Excellent written and verbal interpersonal and communication skills, including professionalism, diplomacy and discretion.

Preferred Qualifications • Master’s degree in social work or public health and/or equivalent experience. • Experience working with local or state public health agency or community health center. • Experience providing technical assistance or conducting trainings. • Demonstrated record of excellent attendance and reliability. • Demonstrated record of adhering to departmental policies and procedures for maintaining confidentiality of all private health information, clinical data, and laboratory information according to confidentiality, HIPAA, and IRB standards. • Experience collaborating with outside stakeholders of various professional levels in a professional and effective manner.
