Purpose: Overall, this position will evaluate programs and partnerships and the progress of strategic plan objectives to assess efficiency and effectiveness of intervention activities. This position is located in the Michigan Department of Health and Human Services (MDHHS), Lifecourse Epidemiology and Genomics Division, Chronic Disease Epidemiology Section.
Asthma Evaluation (0.50 FTE) The consultant will work with the Asthma Epidemiologist, Asthma Principal Investigator and Asthma Program Manager in conducting partnership, surveillance and intervention evaluations for the asthma projects coordinated by the Michigan Asthma Prevention and Control Program (MiAPCP).
Duties and Responsibilities: Specifically, the consultant will: • Consult with asthma program staff in evaluation planning, including evaluation of the surveillance system, partnerships and implementation of interventions. • Update as-needed and implement the MiAPCP Strategic Evaluation Plan, develop and implement individual evaluation plans and share findings with the Centers for Disease Control and Prevention (CDC). • Construct and maintain electronic databases and enter data for evaluation activities. • Develop, implement and monitor data collection, including online surveys, for evaluation projects. • Provide technical assistance for evaluation projects to enhance data quality and completeness. • Participate in cross-state evaluation efforts. • Assist in the collection and reporting of CDC-developed outcome performance measures. • Disseminate findings to CDC and partners, facilitating follow-up action and impact from evaluation projects. • Write interim and summary reports of evaluation projects for MiAPCP as needed and CDC as required.
Deliverables: • Attend MiAPCP, partner and evaluation project team meetings. • Develop and coordinate evaluation plans, survey instruments, electronic databases and program and intervention evaluation reports. • Document action taken from evaluation findings.
Stroke Evaluation (0.50 FTE) The consultant will assist Michigan’s Ongoing Stroke Registry to Accelerate Improvement of Care (MOSAIC) Registry/Program Manager and Principal Investigator in conducting evaluations of system development and quality improvement activities for the stroke systems of care.
Duties and Responsibilities: Specifically, the consultant will: • Consult with MOSAIC program staff and the CDC in evaluation planning. • Monitor MOSAIC progress in meeting grant and system development milestones. • Work with MOSAIC Registry/Program Manager and Data Analyst to monitor performance measures and stroke outcomes to evaluate progress. • Develop, implement and monitor data collection, including surveys of hospitals and other stakeholders, for evaluation projects in the areas of partnership, public awareness, EMS and hospital care, follow up care, integration of systems of care and stroke outcomes. • Provide technical assistance for evaluation projects to enhance data quality and completeness and to assess quality of linkages. • Construct and maintain electronic databases and enter data for evaluation activities. • Conduct basic summary analyses of collected data and provide feedback to QI Specialist and EMS QI Analyst. • Present results to MOSAIC Steering Team, the Michigan Stroke Initiative (MSI) and other partners. • Write interim and summary reports of evaluation projects as needed for MOSAIC staff and CDC as required.
Deliverables: • Develop the MOSAIC Strategic Evaluation Plan (SEP) and annual partnership evaluation report. • Develop and use survey instruments and collect evaluation results. • Develop evaluation materials and evaluation results and present to hospital and EMS staff, MOSAIC program staff, American Heart Association, MSI and other partners. • Develop project and initiative evaluation reports.
