Position Description: The Office of Population Health at New York City Health + Hospitals oversees the development of innovative care models, population health analytics, chronic disease prevention and management, ambulatory care transformation, and implementation research. The Office of Population Health is currently seeking a Grants Manager to serve as the primary grants and contracts manager for the various funding streams in the office and to perform administrative duties to support the functioning of the office and its programs. The Grants Manager reports to the Director of Research and Evaluation and will work collaboratively across the Office of Population Health.

Responsibilities • Oversee the fulfillment of administrative requirements and provide administrative support for federal and state grants • Ensure grant funds are spent according to funding guidelines and relevant regulations • Collaborate with H+H Central Office administration to monitor the progress of grant-funded projects • Develop and maintain project budgets • Develop expense reports and program reports • Manage contracts with vendors • Manage centralized purchases for population health, (e.g., patient education materials) • Provide technical support to sites for grant- related operational issues • Maintain relationships with key stakeholders, including staff at H+H Central Office, clinical leadership and staff at H+H facilities, and external partners • Research and identify new funding opportunities • Other administrative duties as needed

Department Preferences • Four or more years of experience with grants and contracts in government, healthcare, human services, academic research, or a non- profit setting • Experience with all aspects of the grant life cycle, including writing and submitting applications, developing and managing budgets, tracking deliverables, and overseeing reporting • Knowledge of accounting, budgeting, and finance • Strong knowledge of Microsoft Office, including Excel • Strong project management skills; demonstrated ability to develop and manage multiple projects at once, strategize timelines, and meet deadlines • Ability to adapt to new systems and tackle novel problems creatively • Ability to work independently as well as collaboratively with internal and external partners • Demonstrated interest/experience in population health improvement

Qualifications: 1. A bachelor’s degree in healthcare administration, public health, or a related discipline, and 2. Three or more years of full-time paid experience in grants management or a similar area.
