Position Description: As the Health Disparities Coordinator you'll help to fulfill the mission of the Washington State Department of Health Office of Infectious Disease (OID) by identifying and researching health disparities. You'll work collaboratively with OID staff and both known and non-traditional partners across the state to reduce disparities and bring equity to infectious disease health services for affected populations. The position's work integrates with both prevention and care services sections of the Office.

What will you be doing? – Duties include: • Identifying and defining health disparities across the diagnosis, linkage and retention in care, and the successful treatment for HIV, sexually transmitted infections and hepatitis for priority racial and sexual minority populations of interest in Washington State.

• Leading strategic discussions to coordinate the development of plans to address delineated health disparities.

• Engaging medical providers, community based organizations, public health partners and other providers of services to these populations to elicit concrete steps to address and reduce disparities.

• Developing and coordinating special projects to address delineated health disparities by increasing HIV and STI testing among minorities, increasing linkage to PrEP and medical care, facilitating re- engagement for those out of care.

• Improving metrics by addressing social, economic, clinical, and structural factors influencing HIV health outcomes.

• Assisting and coordinating with the special project in Central Washington to address the foreign-born Latinx population.

Qualifications: Required Qualifications

• Option 1: Bachelor's degree or higher in social work, psychology, nursing or a related field; AND one (1) or more years of experience screening individuals for eligibility in public, social service or health care programs; AND one (1) or more years of experience working with government health payers (Medicaid, Medicare and Social Security).

OR

• Option 2: Two (2) or more years of college-level education; AND two (2) or more years of experience screening individuals for eligibility in public, social service or health care programs; AND two (2) or more years of experience working with government health care payers (Medicaid, Medicare and Social Security).

OR

• Option 3: Three (3) or more years of experience screening individuals for eligibility in public, social service or health care programs; AND two (2) or more years of experience working with and understanding government healthcare payers (Medicaid, Medicare and Social Security); AND one (1) or more years providing customer services to the public by phone or in person.

AND

• Demonstrated experience effectively communicating with customers and community partners verbally and in writing.

• Experience effectively working with a team to problem solve, evaluate program effectiveness and effectively resolve conflict.
