Under the supervision of the Program Ma Manager, investigates and monitors reports of of communicable diseases such as measles, he hepatitis, and tuberculosis. Implements pu public health actions as mandated by the pu public health code. Consults with ph physicians, school personnel, and others co concerning the transmission, treatment, and co containment of communicable disease. Pr Provides health care services, education, an and interventions to clients in compliance wi with public health laws. Provides after ho hours availability to ensure seamless access to to public health services and prompt re response to emergency situations. • Investigates, evaluates, and a assesses occurrences of communicable d diseases. Verifies the occurrence and d develops an action plan in compliance with l local, state, and federal directives. • Investigates and evaluates communicable disease occurrences and implements a course of action to individuals and the public health to include assessment, investigation, prevention, surveillance, education, counseling, consulting, enforcement, re-assessment, and i implementation of an action plan. Implements p prevention control measures to protect the p public health. • Monitors and provides case management services to clients with tuberculosis infection in both the private and public health systems. Designs unique plans for clients with complex circumstances such as multi-drug resistant strains of t tuberculosis (TB), language barriers, h housing issues, cultural beliefs, e employment, and economic concerns; ensures p plans address all aspects of the client's n needs. • Administers TB skin tests and interprets results; educates clients on latent TB infection (LTBI) and active TB disease. Co-manages LTBI cases under care of h health department TB clinic, including a administering medications and managing m medication regimens. • Works as a member of the T Tuberculosis (TB) team to investigate, p prevent and control tuberculosis disease. C Conducts active TB case investigations, a assists with laboratory testing, and p performs contact investigations. Works c collaboratively with Colorado Department of P Public Health & Environment (CDPHE) TB P Program. Provides directly observed therapy f for active TB or LTBI cases. • Responds to public health or general i inquiries regarding TB and provides advice t to medical providers, hospitals, and i infection control practitioners regarding m management of TB cases. • Assists with developing and d disseminating training curriculum for d disease prevention and control to local p providers and the community. • Works with local clinical l laboratories, hospitals, infection control p practitioners and healthcare providers to m maintain awareness and compliance of n notifiable disease reporting. • Participates in clinics in all c community settings providing health a assessment, disease prevention and health p promotion activities. • Assesses the impact of local e environmental, cultural and social patterns o on the public's health through collaboration w with service providers and systems. • Assists in policy development by providing expert Public Health Nursing consultation in the community and department. Articulates the mission of public health and develops partnerships to accomplish department goals. Interprets rules and regulations to sub-groups in the c community. • Recognizes patterns of illness i indicative of disease and environmental h hazards. Implements action necessary to c curtail negative impacts of disease. • Serves as liaison between the m medical and public health communities. • Practices within the professional n nursing model as described by the Colorado N Nurse Practice Act. • Participates in departmental e emergency planning and response activities. • Promotes public health within the c community. • Takes personal responsibility to p provide exceptional customer service in o order to promote and maintain a positive D Department of Health image, constructive w working environment and foster pride and p professionalism in the workplace and c community. • Performs other duties as required. MINIMUM QUALIFICATIONS: • Bachelor's Degree in Nursing r required. • Must be a current Colorado R Registered Nurse in good standing. • Ability to communicate, motivate and o organize projects among a broad spectrum of p personnel, frequently under deadline p pressure. Ability to effectively communicate w with the public, medical providers, h hospitals and other community partners. • Skill in demonstrating a c cooperative, professional attitude to c cultivate relationships both within and o outside the organization. • Skill in prioritizing and organizing m multiple tasks and the ability to complete p projects in a timely manner.
