JOB SUMMARY In alignment with the Cook County Department of Public Health (CCDPH) Strategic Plan and under the direction of the Deputy Director, Public Health Programs, the Program Manager is responsible for strategically leading the Chronic Disease Prevention and Health Promotion Unit. This position will work to strengthen the agency's chronic disease prevention control and health promotion initiatives to reduce the burden of chronic disease and promote healthy living and health equity in suburban Cook County. Policy, systems and environmental approaches are key to the Unit addressing the racial, ethnic and other inequities in health outcomes. The Program Manager manages the overall day-to-day operations and personnel of the Unit.

TYPICAL DUTIES • Cultivates and supports linkages or partnerships with other major sectors and key stakeholders in public, nonprofit and private entities to increase awareness, drive action, and ensure accountability in efforts to reduce the burden of chronic disease, promote health and wellness, and address health inequities. • Uses media, social media, advanced technologies, community networks and public relations concepts to communicate information. • Serves as CCDPH’s liaison with local, state and national organizations on matters involving chronic disease prevention/control and represent the Department on advisory boards, work groups, taskforce and consortia for related initiatives. • Develops and ensures implementation of an agency and community- wide strategic plan around chronic disease prevention control, including tobacco prevention and control and health promotion initiatives. • Develops and implements evidence- based interventions, and conduct evaluation to ensure on-going feedback and program effectiveness. • Applies a health equity lens to the development, execution and evaluation of programs. • Translates and disseminates knowledge generated by research and evaluation for decision-making about policies, programs, grant-making, and related equities. • Implements strategies to change policies of organizations and communities capable of affecting the health of suburban Cook County residents, especially those who are most impacted by adverse health outcomes.

Qualifications: • Master's degree in Public Health, Public Administration, Planning or health related field from an accredited college or university. (official transcripts due at time of interview) • Eight (8) years of experience in public health. • Five (5) five years of supervisory experience. • Three (3) years of experience with grant management. • Position requires moderate to heavy local travel for which the employee must possess a valid driver’s license and insured vehicle or otherwise provide an acceptable and reliable means of transportation. • Must obtain ICS 100, 200, 700, 800 certifications within six (6) months of employment.
