Position Description: The Program Manager provides support and oversight for a key Specialty Care Transformation initiative, eConsult. Over the next three years, we will implement an electronic referral and consultation solution across all NYC Health + Hospital facilities, reducing barriers to obtaining specialty care input, and improving the quality of care for our patients.

The Program Manager:

• Is responsible for overseeing the successful delivery of this project, including scoping, prioritizing, and supporting sub-projects while ensuring that top-level deliverables and timelines are met.

• Works closely with the clinical and administrative teams within Central Office and at all facilities, including hospitals, community health centers, and long-term care facilities, to develop policies and standards that facilitate improved specialty access and referral management for our patients.

• Provides ongoing change management and quality improvement support for each clinic, including but not limited to workflow redesign for practice clerical staff, development of new standard work and training materials, and data analysis to facilitate operational changes, e.g. scheduling template modifications. This will include one-on-one support as well as larger trainings or workshops as needed.

1. Graduate degree (MA, MPH, MPA, MBA, MD) with a focus on public health, health care, or related field preferred. 2. At least four years’ experience with progressive responsibility in a program/project management or administration capacity 3. or a satisfactorily equivalent combination of education, training and experience.

Qualifications: Department Preferences

Experience in health care delivery systems redesign and/or quality improvement to support improved operations and patient flow. Experience in using health information technology to support population health and health care delivery; familiarity with Epic preferred.

Demonstrates initiative, sound judgment and flexibility; possesses effective time management and organizational skills; comfortable working under deadlines; has excellent verbal and written communication skills.

Strong interpersonal, communication, and group facilitation skills are essential. Excellent ability to develop relationships and foster trust with diverse stakeholders.
