Position Description: This PHNC2 position serves as the Linkage to Care, Data to Care and Capacity Building Coordinator for the Tennessee Department of Health (TDH) HIV Prevention Program. This Coordinator is responsible for providing oversight to statewide HIV linkage to care, data to care (care re-engagement), and prevention capacity building activities. This position is in the Division of Communicable and Environmental Diseases and Emergency Preparedness (CEDEP), HIV/STD/Viral Hepatitis Program. CEDEP includes a large and diverse group of epidemiologists, subject matter experts and support services, in a wide range of programs.

Job responsibilities include:

Linkage to care (LTC) and Data to Care (DTC) • Oversee the statewide Linkage to Care Program, including development of linkage and re-engagement to care policies and procedures in collaboration with the HIV prevention, surveillance, and Ryan White programs • Sustain partnerships with the HIV medical care community to enhance HIV care management for people living with HIV • Facilitate data-driven discussions and strategic planning with community based organizations (CBOs), Centers of Excellence (COEs), and local health departments (HDs) to improve linkage and re-engagement outcomes • Collaborate with HIV Testing Director, HIV Prevention Epidemiologist, and the Surveillance Program to produce and translate quarterly linkage and re- engagement to care site reports for program evaluation and to inform technical assistance needs with partners • Construct a linkage and re-engagement to care team at Central Office • Perform quality assurance in HIV/STD surveillance systems (e.g., PRISM) to ensure field records for all persons newly diagnosed with HIV are dispositioned correctly • Provide training and direct supervision to TDH Linkage and Re-engagement Specialists(s) • Establish processes for routine reporting of agency-level linkage and re-engagement to care outcomes • Establish a formal data feedback loop for linkage and re-engagement activities • Explore ways to obtain provider-level viral suppression data • Assist CBOs and COEs with verification of linkage to care for patients lost to follow up • Monitor data reporting for re-engagement activities of Linkage and Re-engagement Specialists across Tennessee • Develop reports in REDCap to track the progress of linkage and re-engagement activities • Ensure Not in Care lists are distributed to linkage and re-engagement specialists in a timely manner

Capacity Building • Coordinate and conduct statewide HIV testing and counseling trainings, Antiretroviral Treatment and Access to Services (ARTAS) trainings and PrEP (HIV pre- exposure prophylaxis) trainings for CBOs and HDs • Update and maintain the Prevention Training database, to include those individuals who have completed the testing and counseling and ARTAS trainings • Submit training and capacity building assistance needs through the CDC Clinical research Information System (CRIS) request system • Coordinate and conduct additional educational programs as related to HIV Prevention as needed • Provide technical assistance to certified trainers, CBOs and HDs across the state • Attend meetings including monthly HIV Prevention Program meetings and biannual HIV Statewide meetings • Develop a training curriculum for Linkage and Re-engagement Specialists

Qualifications: Required Education and Experience: Graduation from an accredited college or university with a bachelor's degree in nursing and experience equivalent to four years of full-time increasingly responsible professional nursing work.

Substitution of Experience for Education: Qualifying full-time experience in nursing may be substituted for the required education on a year-for-year basis, to a maximum of two years.

Substitution of Graduate Education for Experience: Additional graduate coursework in nursing may be substituted for the required experience on a year-for-year basis to a maximum of one year.

Necessary Special Qualifications: Must be licensed as a Registered nurse in the State of Tennessee.

Desired Qualifications: • Experience working effectively with diverse groups (e.g. governmental agencies and officials, professional organizations, and the public) • Strong computer skills, including demonstrated use of Excel or other databases to manage and/or evaluate data • Experience conducting public health education/trainings • Experience collecting and managing data • Experience with program management and evaluation • Experience with supervision and training of professional staff • Strong oral and written communication skills • Familiarity with HIV prevention services, disease intervention, and disease investigation • Willingness to travel extensively throughout Tennessee

Must be eligible to work in the U.S. Degrees must be from an accredited college or university. A transcript evaluation is required for degrees earned outside of the United States.
