Position Description: The Medical Transportation Case Manager assesses and addresses clients’ barriers to care, determines appropriate transportation resources available, and coordinates clients’ access to transportation services. The The Medical Transportation Case Manager also work works as a part of an Interdisciplinary Heal Health Team to coordinate transportation to medi medical and social service appointments.

Duties:

•Determine most effective transportation resource and provide linkage for transportation resources to ensure they arrive on time for all appointments by planning and following the most efficient transportation route

•Coordinate MAO Transit schedule with the appropriate care provider for all appointments

•Ensure all clients have a current transportation waiver on file

•Accompany clients to outside medical appointment to facilitate ongoing access to care as assigned

•Possess and utilize a variety of case management techniques to facilitate enhanced client engagement client engagement

•Establish and maintain excellent working relationships with business partners such as Medical Transport of Alabama and the Alabama Medicaid Agency

•Report any vehicle mechanical problems to supervisor and keep van exterior and interior clean •Comply with traffic regulations in order to operate vehicle in a safe and courteous manner

•Accommodate clients with disabilities by loading and unloading wheelchairs, crutches, braces and other medically necessary items

•Demonstrate and relay a skilled knowledge of of HIV / AIDS issues on individuals, families, and special populations

•Maintain daily log of mileage, number of trips, passengers, and destinations

•Participate in continuous quality improvement and outcome evaluation activities

•Provide educational skill building opportunities to foster adherence to medication , improve adherence to health routines, and communication with care providers

•Conducting informal assessments of client’s need for primary care/treatment, supportive services, early intervention/diagnosis information, peer counseling, and assistance in accessing appropriate services •As with all other positions, during emergencies, the Medical Transportation Case Manager is expected to fill in at any position within the agency for which he / she is capable of performing

•Perform any other department / agency related duties or special projects as directed by supervisor

Qualifications: •Must have a bachelor’s degree in either a so social science or health and human services field

•Must have a minimum of 3 years’ experience providing case management (medical and / or non-medical)

•Must have a minimum of 3 years’ experience providing medical transportation (emergency and / or non-emergency)

•Must be CPR certified or obtain CPR credentials within the first 90 days of hire

•Must be able to pass a drug test

•Must have effective auto-insurance coverage and a valid Alabama Driver’s License

•No more than three moving violations in the past three years
