Position Description: Purpose: This clinical subject matter expert position provides clinical consultation and technical assistance in the areas of diabetes prevention and management. Consultation is provided to increase awareness and promotion of Diabetes Self-Management Education and Support programs (DSMES) and Diabetes Prevention Programs (DPP) in Michigan. In collaboration with the DSMES Certification Program Coordinator, this position will be responsible for identifying areas of opportunity for growth, issues related to program access, areas of need, and potential partners or organizations that would help strengthen and sustain DSMES in Michigan.

Duties and Responsibilities:

Management Education and Support programs (DS (DSMES) and Diabetes Prevention Programs (DP (DPP) in Michigan. In collaboration with the DSM DSMES Certification Program Coordinator, thi this position will be responsible for ide identifying areas of opportunity for growth, iss issues related to program access, areas of nee need, and potential partners or org organizations that would help strengthen and sus sustain DSMES in Michigan.

Duties and Responsibilities: • Works collaboratively with the D Diabetes Prevention and Control Program ( (DPCP) in administration of clinical and s support services for people with diabetes or a at risk for diabetes. • In collaboration with the DSMES P Program Coordinator, provide clinical c consultation and serve as an expert resource t to the Diabetes and Kidney Disease Unit p program staff and partners. • Works with DPCP to implement and e evaluate quality improvement processes r relating to data and clinical quality. • Maintains own clinical competency i involving new and emerging technologies for d diabetes prevention and management • Prepares written material, including t technical documents and success stories, for h health professionals and consumers to share b best practices, program guidance, and l lessons learned. • Provides consultation about diabetes management clinical practice to local agencies and clinicians, primarily related to screening and referral to evidence-based p prevention and management programs • Advise and assist with efforts to increase engagement of pharmacists in the provision of medication management and Diabetes Self-Management Education and S Support (DSMES) • With MDHHS Emergency Services C Community Paramedicine program, explore and e evaluate the potential use of community p paramedicine programs • Advise and assist with efforts by t the National Kidney Foundation of Michigan t to begin development, implementation, and e evaluation of emerging screening profiles to d detect chronic kidney disease. • Lead efforts to increase the use of h health information exchanges to increase r referrals to DSMES. • Works with diabetes partners to obtain clinical advice according to national guidelines and the programs’ medical p protocol, as needed. • Provide consultation to partners on c contract development and management. P Participate in site visits and conference c calls to monitor performance and to enhance c collaboration and share lessons learned. • Participate in Diabetes and Kidney U Unit and Diabetes and Other Chronic Diseases S Section staff meetings and represent program o on committees and work groups as requested. • Assist with DOCDS assignments r related to diabetes and kidney disease from M MDHHS Administration, MDHHS Communications, P Population Health and Community Services, M Medical Services Administration and others, a as requested by the Unit Manager. • Contributes to CDC progress reports, c competitive, and continuing applications s summarizing program clinical care a activities. • Attends national meetings as r required by CDC. • Uses knowledge and best practices to b build health equity and reduce health d disparities. • Other duties as assigned.

Qualifications/Requirements:

Education: Possession of a bachelor’s or master’s degree in nursing or related field. A current Michigan license as a registered nurse is required. A Master’s Degree Degree in Nursing, Public Health, or a health healthcare field is also desirable but not requir required.

Experience: Two years of professional expe experience in a position responsible for prov providing clinical subject matter expertise and and implementation or direction of health prog programs including experience in project mana management. Public Health experience work working with vulnerable or underserved popu populations is desirable.

This position requires clinical kn knowledge of chronic diseases including di diabetes, prediabetes and kidney disease; Ex Excellent oral and written communications; Ab Ability to effectively communicate with di diplomacy under challenging circumstances; Ab Able to work well in team situations; Able to to conceptualize changing health care sy systems, and effectively implement and mo monitor new and existing programs; Complete wo work in a timely manner, and handle multiple ta tasks at one time; Proficiency in Microsoft Of Office applications and familiarity with em email/internet browser software.

Work Environment and Physical Requirements: Work location will be at the Michigan Department of Health and Human Services in the Cancer Prevention and Control Section currently at the Washington Square Building, 109 Michigan Avenue, Lansing, Michigan. Job may require moderate physical effort including lifting materials and equipment of <50# and involves viewing a CRT or VDT screen 25% to 75% o f the time. Standard office environment. Requires valid vehicle operator’s license where needed to perform duties of the position. Frequent deadlines. In-state travel for site visits, trainings and/or meetings and out-of-state trave travel may be required.

Responsibility for the Work of Others: Occ Occasional Leader: This position will not sup supervise or manage the work of others but may may be assigned project management res responsibilities and/or be asked to coo coordinate collaborative efforts involving sta staff and other diabetes prevention and con control program partners.

Impact on Projects, Services, and O Operations: This position performs duties a and responsibilities on behalf of the M Michigan Department of Health and Human S Services (MDHHS) and the Diabetes Prevention a and Control Program. This nurse position p provides technical consultation for the c cancer services programs at the Diabetes and K Kidney Unit at MDHHS. While fulfilling t these functions, this person serves as a c critical member of a team of professionals w working to reduce morbidity and mortality d due to diabetes and kidney disease and has d direct contact with many leaders including p physicians, health care facility a administrators, and diabetes professionals.
