Purpose: The Michigan Department of Health and Human Services (MDHHS) Perinatal Hepatitis B Prevention Program (PHBPP)’s mission is to identify hepatitis B surface antigen (HBsAg)-positive women prenatally or at d at delivery for each pregnancy so that their infa infants, household and sexual contacts can be t be tested and treated to prevent the spread of t of the hepatitis B virus (HBV). This posi position will work with multiple entities to assu assure that families are identified, prov provided information to prevent disease comp complications, and ensure completion of medi medical interventions associated with HBV tran transmission.
Duties and Responsibilities: Perinatal Hepatitis B Prevention Case M Manager: • Establish and maintain relationships w with local health departments, maternal s support services, providers, labs, h hospitals, infant care providers and clients e enrolled in program • Identify HBsAg-positive pregnant w women • Identify the infant(s) born to these w women and their household and sexual contacts • Conduct case management to ensure t timely and appropriate prophylaxis is p provided to the infant, household and sexual c contacts • Establish contact via telephone, email or by mail with clients and providers regarding the care and treatment of hepatitis B exposed infants, household and sexual contacts and HBsAg-positive pregnant w women • Contact clients and agency officials t to verify hepatitis B vaccination dates, t tests dates, and results • Maintain a statewide database of h hepatitis B perinatally exposed infants, and h household/sexual contacts • Enter all updates for vaccine a administration, blood test results, d demographic information, provider i information, and client agency contacts • Assure completeness of data entry a and data comparison with various programs i including the Michigan Care Improvement R Registry (MCIR) • Review the Michigan Disease S Surveillance System (MDSS) for completeness o of disease reporting • Generate progress reports, s statistical reports, and annual reports • Assist with updating and d disseminating program educational materials i including program guidelines, Centers for D Disease Control and Prevention (CDC) r recommendations, and other hepatitis related m materials • Assist members of the team with s surveys, chart reviews, site visits, e educational trainings either in person or v via webinars, and evaluation of data • Other duties as assigned
Hepatitis conferences and education: • Assist with conference and e educational opportunities, promotion, p planning, registration, reservations, s scheduling, and distribution of program m materials • Serve as the contact person for all h hepatitis education outreach sessions • Assist in conference goals, o objectives, agendas, and materials • Complete program certificates and corr correspondence for all attendees Qualifications/Requirements:
Education: Bachelor’s Degree in Public Healt Health, Social Work, Health Education or field field related to health, medicine or case manag management.
Experience: A minimum of one to three years of of related and progressively more res responsible work experience in health or med medicine, public health advocacy and edu education, or case management.
Important Skills and Characteristics: • Excellent interpersonal and t telephone skills • Excellent written/oral/presentation c communication skills • Familiarity with immunization issues • Ability to work independently and m manage time efficiently • Proficient in Word, Excel, Access and Pow and PowerPoint
Work Environment and Physical Requirements: Routi Routine physical activities: Standing, sitti sitting, walking, reaching, and bending. RESPONSIBILITY FOR THE WORK OF OTHERS: The Hep Hepatitis B Case Manager will assist the sec section manager and hepatitis B coordinator wit with other duties/projects as needed.
IMPACT ON PROJECTS, SERVICES AND O OPERATIONS: The Hepatitis B Case Manager w will be responsible to represent MDHHS in a p professional manner. This employee will c contact local health departments, private p providers, hospital staff and clients daily. T The contact will help to assess individual n need, provide educational materials, and e establish report with different agencies and c clientele.
