Position Description: Responsible for the administration, operation, evaluation, and fiscal management of the Family Case Management/High Risk Infant Follow-Up, HealthWorks of Illinois, Lead Screening/Prevention, and Immunization programs as well as the direct administration of program personnel.

Qualifications: Bachelor of Science in Nursing (BSN) or Science (BS) from an accredited college or university. Current licensure as a Registered Professional Nurse in Illinois with three to four years’ experience in c community health or maternal/child health n nursing with progressively responsible e experience. Master’s degree in health related field and p previous supervisor experience preferred.
