Position Description: The Virginia Department of Health is looking for an individual to support the execution of the population health approach to health improvement at VDH through leadership of the State Health Assessment and State Health Improvement Plan (SHA/SHIP) process. Conducts and coordinates agency-wide population health analysis, assessment, improvement, and reporting through planning, implementing, and evaluation of statewide needs through data driven methods. Assessment includes the engagement and inclusion of all relevant public health programs and local health districts and relevant state/local community stakeholders. Works closely with relevant staff within central office programs, local health districts, and stakeholders to: 1) design and assure the timely completion of the State Health Assessment (SHA) and State Health Improvement Plan (SHIP; currently the VPfWB), including preparation of reports and annual updates; 2) assure the provision of guidance and unified direction related to Community Health Assessments and Community Health Improvement Plans (CHA/CHIP), to fulfill VPfWB goals and objectives; 3) prepare and/or coordinate relevant grant/cooperative agreement applications; and 4) assure the development and provision of relevant population health training and educational materials. Ensures the collection and quality assurance of multiple program-specific data sources for sound quantitative and qualitative analyses. Coordinates Population Health Action Team meetings with guidance and input from the Deputy Commissioner for Population Health. Leads the VDH SHA/SHIP project team including development of project charters and monitoring of deliverables and milestones. Serves as lead staff to external SHA Advisory Committee and as a point of contact for external stakeholders involved in the SHA/SHIP process. Assures technical assistance to local health districts regarding CHA/CHIP execution and appropriate consistency and quality. May serve as a primary contact regarding SHA and SHIP/VPfWB objectives and activities, including presentations and working with the relevant staff to develop news releases and coordinate media contacts and activities. Assures that the Population Health web page is current and informative.

Qualifications: Knowledge of public health principles/practices; community health assessments/improvement plans (CHA/CHIP); strategic planning/public health accreditation; policy analysis; budgeting; program development; evaluation methods; epidemiologic principles and quantitative/qualitative methods for health improvement; social determinants of health; data management and quality improvement; population-based health improvement strategies; and grants management Skilled in project management; large stakeholder facilitation; health program planning, implementation and evaluation; development of health improvement goals, objectives; and CHA/CHIP; technical/grant writing; effective communications and public speaking; collecting/managing/analyzing/ presenting data as information; conducting population health education training; and MS Office products. Ability to adhere to health assessment/improvement planning for public health accreditation standards; seek/prepare RFPs/grant applications; work independently and as a team; manage effective communications, networks and time management.

Prefer graduate degree in Public Health, Policy, Health Education, or related field, or equivalent training/experience, as well as experience in public health management, policy/partnership development, health improvement planning, and survey development. Health Ed. Specialist cert. helpful. Valid Virginia driver's license and some travel required.
