Purpose of Job The purpose of the position is to conduct field outreach to people experiencing homelessness in Knox County and assisting with the mobile medical unit as appropriate. Essential Functions The Health Outreach Specialist is responsible for the following essential functions:
Conduct field screenings and assessments for SUD, HIV, viral hepatitis, and other conditions associated with chronic substance use.
Conduct rapid HIV, hepatitis C, and other field screening tests as well as perform appropriate pre- and post-test counseling.
Facilitate linkage to and retention in care for clients living with HIV, viral hepatitis, SUD, and other conditions associated with chronic substance use.
Facilitate linkage to community resources for unmet social needs, such as food, shelter, clothing, etc.
Document all client encounters in HMIS as well as in the KCHD electronic health record, where indicated.
Collaborate with homeless service providers, harm reduction agencies, MH/SUD providers, and other agencies as appropriate to provide the highest quality care possible.
Ensure regular, punctual attendance during scheduled work hours.
Respond to and report immediately if called upon by local, regional or central office supervisors, as part of a coordinated emergency response by the Knox County Mayor’s Office
Perform other duties as required
