Position Description: Job Summary

The Violence Prevention Specialist will assist with the CHOP: Violence Intervention Project. This project promotes a “trauma-informed” approach to helping patients who have been admitted to the CHOP trauma service due to an interpersonal assault. The Violence Prevention Specialist will receive training and supervision to conduct community-based case management in partnership with master’s level clinicians. This includes working within the hospital and the community, completing intake and consent forms with families, ongoing face to face contact with clients and families, transporting clients to community resources, and completing weekly patient updates. Additionally, the Specialist will enter patient data into applicable databases, and document all encounters (by phone and face to face).

Job Responsibilities

Identify eligible patients and visit patients and families in the home, school and community

Participate in weekly team meetings, case reviews, and psycho-educational groups

Attend court hearings, school visits, psychiatric evaluations, client therapy sessions, etc. as necessary.

Assist in school transfers and referrals to community resources, e.g. mental health therapy, including arranging for transportation needs

Complete documentation, including intake and consent forms, encounter forms, weekly patient updates, etc. in a timely manner that is legible, clear and organized according to department, hospital and legal standards.

Identify and communicate with hospital and community resources on behalf of patients

Other duties as assigned

Required Education and Experience

A Bachelor’s Degree along with 3-5 years experience as it relates to trauma Preferred Education, Experience & Cert/Lic

Master’s preferred ( MSW) 2 plus years social work experience preferred Previous experience in working with children and diverse populations is an advantage. Additional Technical Requirements

Employee should be comfortable in working throughout the city and must have a valid drivers license, since travel is required. Excellent problem identification and problem solving skills. Excellent written and interpersonal skills. Ability to handle confidential matters and materials. Ability to work with sensitivity to cultural diversity and to assist staff in understanding this diversity
