Position Description: A Viral Hepatitis Linkage to Care Coordinator (VHLtC) is needed to provide contractual assistance for a recently awarded Frontlines of Communities in the United States (FOCUS) grant to the Division of Disease Prevention (DDP) Viral Hepatitis Testing Program. The FOCUS VHLtC will assist local health departments (LHD) and other clinical staff providing hepatitis testing in the coordination of linkage-to- care services for HIV-coinfected and hepatitis C virus (HCV)-monoinfected patients in the southwest and northwest regions of Virginia. The FOCUS VHLtC will work onsite at the VDH central office in Richmond, VA and provide assistance related to FOCUS project activities as needed. The FOCUS VHLtC will work closely with the FOCUS Project Coordinator, the state VHLtC, the Viral Hepatitis Epidemiologist, and the Viral Hepatitis Testing Coordinator. The FOCUS VHLtC will recommend and implement plans for improving linkage-to- care processes; establish protocols; identify and ameliorate barriers; and prepare reports documenting findings. The FOCUS VHLtC will work with the state VHLtC to develop a comprehensive referral directory of statewide resources to ensure that testing programs have an up to date referral list for HCV treatment providers. The FOCUS VHLtC will ensure appropriate training is provided to district personnel and other internal/external individuals and groups performing testing activities where linkage to care services are needed.

Qualifications: Minimum: Knowledge of public health practices; epidemiology and disease intervention methodologies (particularly those related to hepatitis and/or HIV/STD). Knowledge of relevant laws, regulations and procedures related to disease reporting and control. Strong oral and written communication; interpersonal; program implementation and management; and computer (especially Word, Excel and PowerPoint) skills. Ability to interpret viral hepatitis epidemiologic data; conduct training; prepare reports; make sound decisions; provide technical assistance and training. Ability work independently and as part of a team. The FOCUS VHLtC should have a degree in a health-related field and/or related training and experience. A valid Virginia driver’s license required. Preferred: Advanced degree in a health related field; knowledge of state procurement and contracting guidelines; grant writing experience; ability to analyze epidemiologic data and conduct program evaluation.
