Position Description: |
Michigan Medicine Department of Internal Medicine and the Michigan Social Health Interventions to Eliminate Disparities Collaborative Quality Initiative (MSHIELD CQI) seek an Social Drivers of Health Manager to help to lead MSHIELD's community-clinical linkage work that implements screening for unmet social health needs in healthcare settings and links patients to existing resources in their communities. The SDOH Manager facilitates the development of social needs screening-linkage interventions, including related activities of evaluating interventions, maintaining key partnerships, and offering consultation and education to the CQI portfolio, in close coordination with other team members. The position also includes key duties related to capacity building among our organizational partners and maintaining strategic partnerships in Michigan's communities.

Qualifications: |
Required Qualifications Demonstrated experience leading relevant projects with multiple partners or stakeholders.
Excellent organization, communication, and interpersonal skills.
Ability to work in a fast-paced environment, handle multiple projects simultaneously, set priorities, and take initiative while keeping supervisors and project leads informed.
Ability to be consistently detail-oriented.
Flexibility and ability to work independently and collaboratively with multiple teams and individuals.
Proficiency in basic Microsoft programs including Word, Excel, and PowerPoint.
Senior Level
Bachelors degree or higher in health sciences, public health, social sciences, or a related field.
Four or more years of progressively responsible experience in a relevant setting.
Two or more years of experience in a healthcare, social service, community-based organization or related setting.
Intermediate Level
Bachelors degree or higher in health sciences, public health, social sciences, or a related field.
Two or more years of responsible experience in a relevant setting, per above.
Desired Qualifications Masters degree in health sciences, public health, social sciences, or a related field.
Experience leading or facilitating implementation of social needs screening and linkages in healthcare settings
Experience working with or in community-based organizations in Michigan
Ability to work effectively with limited supervision.
Experience with health equity, anti-racism, or supporting the health of diverse communities.
Commitment to personal and professional growth, particularly related to health equity and anti- racism.
