Position Description: • Working with Atlanta area hospitals and their social work and care coordination teams training them to understand resources available for their soon-to-be discharged patients who are experiencing homelessness so that they are discharged to a warm bed instead of to the street. • Developing a good working relationship with Atlanta-area shelters understanding their needs, understanding the types of individuals they house, their programs and understanding bed availability. • Developing an on-line resource guide to help hospital social workers and care coordination teams navigate the ability to find a shelter with an available bed for their patient and their patient’s needs upon discharge. • Developing an on-line web page with real- time bed availability as well as working with shelters to get their bed availability on- line in real-time. • Understanding the HMIS software ClientTrack used for data entry for those individuals experiencing homelessness.

Qualifications: • At least a Bachelor’s Degree; Master’s Degree preferred • Creative, self-started, ability to work independently • Strong project management skills • Significant experience building relationships, and gaining community buy-in and consensus building • Previous experience in partnering with leadership for the delivery of projects • Excellent client-facing and internal communication skills • Solid organizational skills including attention to detail and multi-tasking skills • Preferred record of measurable results in cultivating relationships • Superb organizational skills, with the proven ability to create and implement efficient development systems, plan strategically, allocate work, set and manage objectives, analyze and mitigate risk, and create and manage projects • Ability to excel in an entrepreneurial, fast-paced, diverse, results-oriented environment, and to handle multiple projects simultaneously under tight deadlines • Excellent verbal communication, interpersonal, and relationship-building skills to effectively work with a broad spectrum of stakeholders • Impeccable, professional, and persuasive writing and editing skills • High energy and a passion for the strategic direction of ending homelessness • High emotional intelligence with the ability to perceive and understand people’s motivations • Highly industrious with a strong work ethic; sense of urgency/impatience for action • Integrity and humility that encourages and inspires constituents; strong sense of judgment • Ability to seek out constructive criticism and growth • Strong working knowledge of Microsoft Office and of project management tools

Organization Description: Non-profit Individual would be a part of a larger group made up of administrators at five Atlanta hospitals that works with permanent supportive housing and shelters throughout the City of Atlanta.
