Position Description: The Coordinated Intake Supervisor gives oversite to the Maternal, Infant, Early Childhood Home Visiting Program – MIECHVP (iGrow) ensuring a single point of entry for access, assessment and referral to family support services in Winnebago County. The Coordinated Intake Supervisor will work with the identified agencies in the community to provide support and guidance in developing an effective community system to ensure early and periodic contact with all families of children, beginning during pregnancy and continuing to age five. The Coordinated Intake Supervisor screens families to assess their strengths and needs and refers them to participating MIECHVP agencies. The Coordinated Intake Supervisor serves as a liaison to MIECHVP agencies, referral sources, families, team members, and community agencies. Is responsible for coordination of outreach staff to inform and educate the community including healthcare systems, schools, social service agencies, and others regarding MIECHP services and secures agreements with referral partners.

ESSENTIAL FUNCTIONS:

Manages the MIECHV Program and staff

Supports the Department’s application and preparation efforts for national public health accreditation.

Collaborates in developing a work environment where continuous quality improvements in practice are pursued.

Assumes responsibility for own professional growth and development by pursuing continuing education participating in professional committees and work groups.

Qualifications: Education, Licensure, Certifications, Experience

Bachelor’s Degree required. Bachelors Degree in a Social Service-related field, Health Education, or Community required. Master's Degree in Public Health or related field preferred.

Minimum of 3 year’s supervisor experience required. Previous home visiting experience required. Previous program management experience required.

Preferred minimum two (2) years’ experience working with federal or state grant deliverables.

Experience in collaboration building, collaborative structures, community systems, and systems building across diverse early childhood programs and services.

Demonstrate strong interpersonal skills, including the ability to engage both traditional and non-traditional participants

Knowledge of best practices in trauma- informed organizations and trauma-informed care

Must participate in assigned trainings, including on-line training in the National Incident Management System (NIMS) offered through the Federal Emergency Management Agency to obtain the following certifications:

Within 6 months of employment: ICS 100, ICS 200, ICS 700, and ICS 800

Within 12 months of employment: ICS 300 and ICS 400

Successful completion of WCHD HIPAA Training within the introductory period.

Valid driver's license, current certificate of vehicle insurance, and access to reliable transportation to all assigned work locations
