Are you passionate about driving community health improvements through data-driven strategies and collaborative engagement? The Pima County Health Department is seeking a Population Health Coordinator to support and implement strategic initiatives aimed at improving the overall health and well-being of our community.
In this role, you will lead the implementation and management of the Healthy Pima initiative, which includes the community health improvement planning process and the Healthy Pima Indicators project. Your work will focus on transforming data into action, fostering innovation, and promoting engagement among internal teams, external stakeholders, and community partners to address key public health challenges.
This position plays a critical role in aligning department efforts with the Community Health Needs Assessment (CHNA), accreditation standards, and performance and quality improvement initiatives. A strong foundation in qualitative data analysis and community engagement is essential, as you will support teams in making data- informed decisions that shape programs and interventions across the department and Pima County. Essential Functions:

Coordinates program activities, timelines, and resources to ensure efficient execution;
Collaborates with program managers to develop and maintain project plans and work closely to meet program goals and objectives;
Tracks program progress, milestones, and deliverables, including grant contracts;
Collects, organizes, and analyzes data related to program performance, outcomes, and impact;
Assists in developing data collection tools, surveys, and assessment instruments;
Supports community outreach efforts and participates in public health education and awareness activities;
Leads community engagement efforts, including outreach, needs assessments, and partnership development;
Assists in budget management, expense tracking, and procurement activities;
Identifies areas for program improvement and assists in implementing corrective actions;
May supervise support staff and execute performance plans and individual development plans.
