Examples of Work With an emphasis on prevention and assisting individuals to remain independent within the community, conducts comprehensive, community- based health assessments Provides case management and care coordination services to older adults per DHS and health plan guidelines Works as a resource to individuals and their families in order to help them understand the implications of medical diagnosis and support them with carrying out treatment plans Arranges services needed by clients and coordinates the delivery of these services Conducts assessments of individuals to determine eligibility for public funded programs Makes referrals to service providers as appropriate Provides appropriate health promotion and education information to clients May assist epidemiology disease surveillance, investigation activities, and participate in public health disaster preparedness Performs related duties as assigned

Required Special Qualifications:

Licensure as a Registered Nurse in the State of Minnesota is required upon employment. Certification as a Public Health Nurse in the State of Minnesota is required within six (6) months of employment.
