Position Description: The Heart Disease and Stroke Prevention (HDSP) Program Health Educator assists in planning, developing, coordinating, implementing and evaluating evidence-based heart disease and stroke prevention initiatives, with a primary focus on achieving goals and objectives as required by federal and other funding agencies designed to reduce the burden of cardiovascular disease in the state. Additionally, the Health Educator develops and monitors professional services contracts and provides technical assistance, including: developing or linking people/organizations to resources and coordinating statewide activities.
