The Nurse Family Partnership (NFP) Public H Health Nurse is responsible for providing c comprehensive public health nursing services t to clients and their families per e established protocols. Assists clients and t their families in establishing goals and o outcomes. Provides support and referral r resources in assisting clients in attaining t their targeted goals. Evaluates client p progress toward started goals. • Provides home visits to eligible c clients and their families in accordance w with program and department guidelines. • Assists clients and their families i in establishing goals and outcomes. Provides e education, support and referral resources in a assisting clients in attaining their t targeted goals. Evaluates client progress t toward started goals. • Develops and maintains community r relationships to support client referrals. • Assesses the impact of local e environmental, cultural and social patterns o on the public's health through collaboration w with service providers and systems. C Collects, compiles and analyzes data to i identify needs and service gaps for i individuals, families and communities and i informs the agency of the need. • Articulates the mission of public health and develops partnerships to accomplish department goals. Interprets rules and regulations to sub¬groups in the c community. • Recognizes patterns of illness i indicative of disease and environmental h hazards. Implements action necessary to c curtail negative impacts of disease. • Practices within the professional n nursing model as described by the Colorado N Nurse Practice Act. • Participates in departmental e emergency planning and response activities. • Promotes public health within the c community. • Takes personal responsibility to p provide exceptional customer service in o order to promote and maintain a positive P Public Health image, constructive working e environment, and foster pride and p professionalism in the workplace and c community. • Performs other duties as required. MINIMUM QUALIFICATIONS: • Bachelor's Degree in Nursing r required. • Must be a current Colorado R Registered Nurse in good standing. • Bilingual preferred. • Ability to communicate, motivate, a and organize projects among a broad spectrum o of personnel, frequently under deadline p pressure. • Skill in demonstrating a c cooperative, professional attitude to c cultivate relationships both within and o outside the organization. • Skill in prioritizing and organizing m multiple tasks and the ability to complete p projects in a timely manner. • Ability to respond in a flexible m manner and reprioritize work as situations c change. • Ability to demonstrate sound j judgment by taking appropriate actions r regarding questionable findings or concerns. • Ability to identify problems and w work creativity to resolve them, considering t the impact of actions on Public Health. • Ability to identify results of e efforts and problems identified/potential p problems and to develop and recommend c corrective actions to supervisor. • Knowledge and understanding of c company policies and procedures. • Ability to write and maintain a accurate records and reports to meet m management objectives. • Ability to maintain the security of s sensitive and confidential information. • Ability to work independently and in a a team environment. • Skilled in training and p presentation. • Ability to perform under stress and w when confronted with persons acting under s stress. • Ability to work in an environment w with potential exposure to pets, second hand s smoke, and other environmental hazardous. • Ability to work in an environment w with potential exposure to infections, b blood, and contagious diseases. • Ability to travel using own t transportation on a daily basis. • Skill in using a personal computer a and various software packages including M Microsoft Office. • Must pass conditional post offer bac background investigation, physical exa examination, drug screen and motor vehicle che check. LICENSES/CERTIFICATES: • Must possess and maintain a valid C Colorado Driver's License. • Must possess a current Colorado N Nursing License. • Must possess and maintain a valid CPR CPR Certification.
