Position Description: Public Health Solutions’ Access to Health and Food Benefits Program in the Public Health Programs Division helps individuals and families obtain free or low-cost health insurance coverage and apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps). PHS’ Benefit Counselors help families submit applications to HRA for review and eligibility determination and follow up to ensure the applications are processed accurately. They also provide referrals to a wide range of other services, including legal and financial assistance, WIC, and health insurance assistance. Over the past five years, the program has helped over 17,000 people apply for SNAP benefits. Our program uses a model of co-locating staff at trusted community-based sites where eligible individuals and families already receive services, including Public Health Solutions’ Neighborhood WIC Centers, Center for Health Equity’s Neighborhood Health Action Center, and NYC Health + Hospitals. ?

?Responsibilities:

Provide in-person assistance and education to potential enrollees about SNAP. Screen potential enrollees for SNAP eligibility and provide information in a manner that is culturally and linguistically appropriate to the needs of the population. Assist eligible clients with SNAP applications; securing necessary supporting documentation; provide assistance at renewal and additional information as required. Utilize POS properly to submit SNAP applications, recertification, and documents when appropriate. Conduct weekly outreach in her/his respective area as determined and approved by supervisor. Travel within NYC may be required to do cover different sites and events Responsible for meeting weekly targets of accurately completed applications; following up with families regarding the status of their applications as well as resolving problems and answering questions that arise during the coverage period. Responsible for meeting goals of POS submissions and acceptance. Participate in mediation program to assist individuals who encounter problems with their application. Enter data timely and maintain all data in New Technology. Responsible for tracking referral. Handle phone calls. Maintaining a working knowledge of SNAP eligibility rules. Maintain a clean and orderly site. Assist the Director and supervisor on special projects as needed.

Qualifications: Qualifications:
