Position Description: Overview THIS WORK MATTERS!

Communicable Disease Services is a unit within the Public Health Division of the Multnomah County Health Department. Our unit provides communicable disease prevention, treatment, screening and education services in collaboration with community partners. Within CDS, the Investigations and Case Management Team provides rapid response to reportable diseases, including case investigation, case management, outbreak response and implementation of preventive measures to control disease spread in the community.

As the Communicable Diseases Community Health Nurse you will:

Carry out the mission through using the nursing practice model to deliver immunizations, prophylaxis, disease screenings, case and outbreak management, health education/training and consultation, epidemiological surveillance and investigation, and participate in special projects.

Qualifications: Required Minimum Qualifications/Transferable Skills*:

Possession of, or ability to obtain, an appropriate and valid Oregon driver's license Possession of, or ability to obtain, an appropriate and valid license to practice as a Registered Nurse issued by the State of Oregon Possession of current American Heart Association BLS/CPR card, or ability to obtain BLS/CPR within 30 days of hire A Bachelor's degree in nursing from an accredited nursing program; OR An Associate's degree in nursing from an accredited nursing program and two years of registered nurse level experience is required Preferred Special Qualifications/Transferable Skills*: Working knowledge of principles, practices and procedures of clinic patient care Ability to anticipate patient and provider needs Masters in Public Health or other relevant field Working knowledge in how to access, interpret and apply computerized client data information Working knowledge of programs and services available to clients Ability to effectively screen and gather required information in a professional, culturally competent and customer service oriented manner Ability to communicate clearly and concisely, both orally and in writing Ability to establish and maintain cooperative working relationships with those contacted in the course of work Ability to be flexible and manage multiple priorities Ability to maintain composure and work objectively with clients who may be hostile or verbally abusive *Transferable skills: Your transferable skills are any skills you have gained through education, work experience (including the military) or life experience that are relevant for this position.
