Position Description: Under direct supervision within Health, Housing, and Human Service Administration, supports the implementation of effective and efficient division-wide processes and s systems that include: population data m management systems; community planning, o outreach and engagement; and, health and s social equity efforts. Assists in i identifying data measures and conducting b basic statistical analysis and reporting to a assess the population's health and wellbeing. -Assists in the analysis of population-level d data, identifies challenges, determines c continuous improvement strategies, o operational and strategic including the use o of data to drive decision making and p prioritization to guide consensus action p planning -Assists in statistical analysis of data and p prepares narratives and reports to depict g gaps, errors and trends -Assists in the development, modification a and implementation of continuous quality i improvement efforts, including processes, p policies and procedures -Maintains and may assist in the development o of effective community strategic planning p processes, including the development of p performance measures -Assists in making recommendations and uses q quantitative and qualitative tools in c community population data management -Assists in the development of indicator d data and recommends quantitative and q qualitative tools for data collection -Assists in implementing solutions to a address opportunities, in collaboration with m management -Participates on various committees to p provide continuous improvement within the d division -Assists with divisional accreditation efforts; obtaining and maintaining accreditation status

Qualifications: Bachelor's degree from an accredited college in pu in public/community health, mathe mathematics/statistics, social work, public admin administration, planning, business admin administration, process engineering, program plann planning and evaluation or related field and one ( one (1) year of experience in a similar posit position as listed in the work functions OR A combination of education and experience equ equivalent to five (5) years of experience in in a similar position as listed in the work fun functions Desirable Qualifications Master's degree from an accredited college or university in public/community health, mathematics/statistics, social work, public administration, planning, business administration, process engineering, program planning and evaluation or related field or university in public/community health, mathematics/statistics, social work, public administration, planning, business administration, process engineering, program planning and evaluation or related field
