Position Description: 60% Clinical Duties • Provides direct care in the health center, home, mobile clinics or community • Interviews clients and records medical/psychosocial information in electronic medical records • Assists in controlling patient flow, explain clinic procedures and prepare patients for exams • Determines eligibility and screen patients for Vaccine for Children and Adult Safety Net Programs • Draws blood samples, tests urine, screens vision and hearing, takes and records vital signs, heights and weights to assist with patient screening as needed. • Assists with follow up on abnormal test results and missed appointments by telephone or home visit • Participates in performing quality assurance and checks on lab specimens and preparing specimens for transport • Assists in preparing statistical reports • Maintains confidentiality of client’s information • Maintains knowledge of CDC recommended vaccines and administers age appropriate immunizations as required • Enhances and encourages creativity to continue to develop a cutting edge community outreach plan • Accurately reads shot records in a timely and efficient manner and recommends appropriate vaccines to administer to adults and children • Administers vaccines and other prescribed medications according to Pharmacy D Regulations and under the supervision of the Midlevel Provider and Physician • Ability to accurately record patient information in the patient medical record and to notify appropriate personnel when deviations from the norm occur • Basic math skills are required to accurately measure medication doses, weights, lengths, and count vaccine/pharmacy inventory • Ability to draw blood and to administer oral and injectable medications • Maintains confidentiality of the patient’s medical and personal information • Assists with clerical tasks such as daily/monthly reports and patient record processing • Reviews patient record forms for accuracy and completion • Checks statistical calculations to ensure accuracy • Assists in collecting/compiling data to aid in program evaluation • Answers the telephone and give appropriate information • Assists with inventory/replenishes needed supplies. Arranges supplies in order of expiration • Acts as an alternate immunization coordinator for the Department of State Health Services Vaccine For Children Program • Expands and strengthens referring provider network • Provides information by teaching classes or counseling patients on chronic and infectious diseases • Assists in orienting new staff, nursing students or nurse volunteers • Assists with vaccine and pharmacy inventory control • Arranges dated supplies in order of expiration • Replenishes supplies in work area • Adjusts/positions equipment such as scales, exams tables, classroom furniture, projector/screen, and TV/VCR • Uses various instruments such as microscope slides, hemocue microcuvettes, or audiometer to prepare for service delivery • Performs cleanup and sterilization/disinfection of technical areas/equipment • Other duties as assigned, including special tasks involved in responding to an emergency event • May also include working on agency or clinic special projects

Qualifications: Graduation from an accredited school of vocational nursing • Current LVN license from the Texas Board of Vocational Nurse Examiners • Current CPR certification for Health Professionals • Valid driver’s license and reliable means of transportation for outreach activities • 2 years of experience as an LVN in a public health or community health setting • Proficient in utilization of a personal computer with basic data entry skills and experience with email

Organization Description: Under the supervision of the Mobile Health Team Lead, and with oversight and guidance from the Health Transformation Manager, the DSRIP Mobile Health LVN will be responsible for performing assigned nursing care/field task and providing preventive health care/services in the clinic, home or community setting.
