Position Description: Under the direct supervision of the Manager of the Chronic Disease Epidemiology (CDE) branch, performs evaluation activities for chronic disease prevention and health promotion interventions. Oversees and provides technical assistance and consultative services in evaluation to community chronic disease intervention partners. Responsible for monitoring community intervention projects and for developing evaluation and monitoring tools, summary reports on findings and other related analyses and reports. Works with CDE staff and across the Health Promotion and Chronic Disease Prevention (HPCDP) Section to identify appropriate evaluation methods and resolve evaluation-related problems, re recommend changes, oversee initiation and de development of new data sources, and assist in in development of action plans for program im improvement. Works with considerable la latitude for the use of initiative and in independent judgment.

35% Performs the evaluation of chronic disease prevention and health promotion interventions including but not limited to Obesity Prevention Program program, providing direct support and technical assistance for project implementation. Evaluate partnerships and collaboratives with clinical organizations. Coordinates closely with clinical partner organizations, intervention designers and implementers for data planning, identifies data sources, and manages and analyses qualitative and quantitative data for evaluation purposes. Provides frequent and direct feedback to intervention staff on project strategies and activities. Monitors and evaluates research findings relative to evidence-based chronic di disease interventions in clinical settings. De Develops protocols, project timelines, needs as assessments, statewide surveys, conducts in interviews and focus groups related to the Ob Obesity Prevention program. Analyzes data co collected through surveys, focus groups and wr writes reports.

30% Provides expert technical assistance and consultation and coordinates evaluation of the HPCDP section’s chronic disease program interventions. Designs and disseminates evaluation-related support materials such as lo logic models, evaluation frameworks, de develops surveys, conducts focus groups, and cr creates reports. Initiates evaluation pl planning, development, implementation and re reporting of chronic disease interventions in in collaboration with HPCDP programs and ex external organizations. Coordinates closely wi with CDE and intervention staff for data pl planning, identification of data sources, an and collection of data for monitoring pu purposes. Assists in writing grant ap applications by creating evaluation plan and fr framework for proposed programmatic ac activities.

20% Establishes and manages performance monitoring frameworks using well-defined goals, outcomes, outputs, inputs, processes, indicators, data needs, and data sources. Monitors and evaluates overall progress, including long-term sustainability of the inte intervention. Works with CDE staff to reco recommend appropriate changes and assist in deve development of action plans for program impr improvement.

10% Prepares formal contributions to public he health sciences, including conference ab abstracts and manuscripts for publications. At Attends trainings, conferences and workshops to to maintain skills and knowledge related to pu public health science.

5% Performs other duties as assigned. Other duties as assigned also include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response a and/or recovery or Continuity of Operations ( (COOP) activation. Such participation may r require an alternate shift pattern a assignment and/or location.

Qualifications: Knowledge of public health, health pro promotion, and chronic diseases; Knowledge of evaluation methods and tools, in including SMART objectives, performance in indicators, logic models, data collection to tools, and analytic methods;

Excellent interpersonal skills and effective problem-solving skills; Skill in communicating clearly and concis concisely; Skill in building effective partnerships Skill in leading group efforts and working in a in a team environment; Skill in developing procedures to monitor and and evaluate progress in achieving project obj objectives; Skill in conveying planning alternatives or re recommendations to a variety of audiences;

Skill in using MS Office, statistical software (e.g. SAS, SPSS, Stata, NVivo, Atlas.ti Atlas.ti etc) analyzing qualitative and quant quantitative data

Ability to plan and manage complex projects; Ability to provide strong customer service an and maintain positive partner relations; Ability to establish program goals and o objectives that support strategic plans; Ability to create formal contributions to public health science and participate in the peer-review process
