Position Description: End of life decisions are difficult for most families for a variety of reasons, including poor advance planning and complex family communications. Most people have little or no understanding of the end of life treatment options, advance care planning, life sustaining treatment and hospice. They are not aware of their healthcare rights and of the laws that protect them. There are major misconceptions about hospice, palliative care, advance directives, and patient rights. Additionally, demographic differences appear to play a role in end of life care. Ethnic minorities have lower rates of completing advance directives due to a lack of knowledge and understanding by the patient and family about the advance directive and communication challenges between patients and medical staff.

The need for information about existing programs and services is crucial to increase awareness, reduce misperceptions, and improve utilization, and alleviate disparities in care at the end of life. We are seeking a part-time End of Life Community Educators for our Speakers Bureau to provide presentations In New York (New York City and Long Island). All Community Educators will receive training to provide our 45 minute presentation called “Planning for a Better End of Life”. Community Educators will be paid per presentation.

Job description:

· Under supervision of the Director of Outreach Education, deliver an engaging presentation about end of life issues for older adults and caregivers.

· Present 5-8 presentations in the New York City and Long Island in 2020 at senior centers, libraries, community organizations, retirement communities etc.

· Occasionally help Director of Outreach and Education with outreach efforts and coordinate speaking engagements at senior centers, libraries, and other community organizations.

· Collect evaluations and provide summary reports for each presentation.

· Meet monthly (via phone or in person) with the Director of Outreach and Education to review and discuss the presentations, scheduling, and necessary updates.

· Attend a half day training on the content and workshop facilitation.

Qualifications: Job requirements:

· Ability to provide one - two presentations a month as needed and to travel to the boroughs of NYC and/or Long Island (presentations will be based on Community Educator’s schedule, some presentations may be during the day or in the evening)

· Excellent communication skills (both written and verbal), interpersonal skills and the ability to interact with people from diverse backgrounds and environments.

· Experience in public speaking, teaching, and/or facilitating educational workshops.

· High energy and motivation.

· Independent work skills.

· Solid work ethic, attention to detail, and strong time management skills

· Background in social work, nursing, public health, medicine, gerontology, and/or law is preferred.

· Proficient in computer software applications, Word, Excel, and Power Point.

· Experience in conducting programs related to health, navigating health, or social service systems preferred.

· Bilingual is a plus (Spanish, Chinese, Korean, Russian, Creole).
