Position Description: Our Communicable Disease Division is recruiting for a Health Promotion Coordinator II to be responsible for providing coordination, facilitation, and overarching support to PPH project and stakeholders team in implementing strategies and activities to address potentially preventable hospitalization in targeted communities. This is a Project, (1.0 FTE) 40 hours per week Job Assignment.

*This is a grant funded position through June 2020*

Some of what you'll do: •Provide coordination to Potentially Preventable Hospitalizations project. •Help plan, develop, prioritize, and implement program strategies to meet PPH project goals and objectives. •Participate in the research, development, assessment, implementation and evaluation of PPH strategies to ensure alignment with programs and/or cross-divisional goals and objectives. •Assist with convening community partner meetings and stakeholders, and provide project planning and support to participants •Lead and coordinate work groups of PPH strategies to meet goals and objectives. •Provide support for project communications and logistics. •Assist with tracking project evaluation metrics and developing communication plans and implementing tactics. •Assist with quality assurance work to ensure effective and efficient use of resources and management of project activities. What you bring to the table: • Knowledge of health care systems and how people access clinical and behavioral health care. • Knowledge of social determinants of health and evidence-based policy, systems and environmental change strategies • Knowledge of effective health promotion education principles, practices and methods. • Knowledge of social science assessment, intervention, and consultation principles and practices. • Knowledge of basic budgeting and accounting principles. • Skill and ability to lead and coordinate work for teams of professional and technical staff. • Skill in researching, analyzing, recording, and assessing data, i.e., statistical data, health literature, marketing & educational strategies, grant applications, budgets, etc. • Skill in organizing work activities to meet established objectives. • Skill in communicating effectively in writing and orally to individuals and groups. • Skill in the development and presentation of effective health promotion campaigns in a variety of settings. • Ability to establish and maintain collaborative relationships with a diverse population of community members, media personnel, businesses, schools, community- based agencies, organizations and departmental staff.

Qualifications: Bachelor's degree and three years of relevant experience in public health, health care or other related field, or the equivalent combination of education and experience to perform the essential functions of work. A Master's degree is preferred.
