Position Description: The Public Health Representative II will be located in the Bureau of HIV/STD Field Services (BHSFS) and will assist in improving health outcomes among communities heavily impacted by HIV and HIV/STI comorbidities. The incumbent will conduct confidential investigations of persons and their partners based on networks including but not limited to viremic Hispanic and Latino Men who have Sex with Men (HLMSM), MSM, and Black women including daily work in the community within their assigned NYS regional area. The incumbent will also conduct confidential investigations in response to HIV transmission time-space and molecular clusters.

The incumbent will be required to perform venipuncture or fingerstick, conduct disease screening activities in the field, incorporate evidence based interventions into internet-based and face-to-face interactions, contact health care providers to discuss patient care status, link patients newly diagnosed with HIV into care, identify partners and their social networks, complete the required disease investigation forms, enter forms into existing NYSDOH data systems, and other related duties as assigned. The incumbent will also assist in activities related to Out of Care efforts for patients with no evidence of prognostic lab results reported to NYSDOH and ensuring patients diagnosed with HIV are re-engaged into care.

Travel, up to 75%, will be required.

Qualifications: A Bachelor's degree in a related field and one year of experience providing direct patient/client services in a health, human service or community-based organization, or institutional setting; OR an Associate's degree in a related field and three years of such experience; OR five years of such experience. A Master's degree in a related field may substitute for one year of such experience.

A valid driver's license that is in good standing. The incumbent(s) hired into this position must live within commuting distance to the official work location. The incumbent(s) who relocate to New York State (NYS) for this position will be required to obtain a NYS driver’s license. NYS requires new residents to obtain a license within 30 days of becoming a resident.

This position requires that the incumbent will report to the official work location and live within commuting distance to the official work location. Telecommuting may not be available. If telecommuting is available, the incumbent will be required to have internet access.

Preferred Qualifications At least one year of experience in field- based outreach and or community based services; at least one year experience with disease intervention collaboration with community members; at least one year of HIV testing experience; work experience and/or professional knowledge of HIV and Sexually Transmitted Infections; experience working in ethnically and culturally diverse communities; experience educating and or counseling clients in risk prevention / harm reduction related to personal health; social media networking skills; and knowledge of public health principles for communicable disease epidemiology; bilingual in English and Spanish.
