Purpose of Job
The purpose of this classification is to serve as a resource planner to assess the health needs of certain populations and communities, analyze and evaluate programs and policies and use this information to develop plans to carry out public health objectives.
Essential Functions
The Health Planner is responsible for the following essential functions:
Facilitate the Community Health Council and the development and implementation of a Community Health Improvement Plan
Serve as the architect of the health department’s five-year Community Health Assessment
Receive and research input concerning expressed health problems which might require Health Department resources
Analyze information to determine those problems needing resources and make recommendations in relation to the magnitude of problems and risks to the public
Identify and rank potential program solutions with consideration to effectiveness and cost
Compile information on resources, funding, facilities, equipment and staff resources available for various programs
Assist with identifying evaluation methods for program implementation
Assist with plans for departmental program development and funding
Participate in grant development; prepare and maintain a variety of records appropriate to health planning and related financial applications
Strategically foster partnerships with community stakeholders in order to enhance program and grant development opportunities
Work in a variety of programs within the Health Department Respond to and report immediately if called upon by local, regional or central office supervisors, as part of a coordinated emergency response by the Knox County Mayor’s Office
Ensure regular, punctual attendance during scheduled work hours
Perform other duties as required
