The Pima County Health Department is seeking a CMHAi Tribal Case Manager to provide coordination of care, referrals, and screenings, and to help determine eligibility for individually based medically necessary services. This grant-funded position will focus on working with Indigenous peoples and communities in Pima County to bolster the wellness and health outcomes and to reduce substance use-related injuries and fatalities. This position has an opportunity for systems-level advocacy and will work closely with other community service providers, including crisis services, outpatient, housing, the justice system, first responders, and various community resource agencies. Ideal candidates must display strong interpersonal communication skills, organizational skills, and be attentive to detail. This position operates within the Community Mental Health, Addiction, and Injury Division (CMHAi) and under the direct supervision of a Public Health Program Coordinator.

Conduct thorough assessments of clients' medical needs, considering physical, mental, and emotional health; additional assessment should consider the social determinants of health.
Develop a comprehensive understanding of clients' health histories and current health status.
Develop individualized care plans in collaboration with clients, healthcare providers, and other stakeholders.
Set health goals and strategies to address clients' medical needs and enhance overall well-being.
Collaborate with healthcare professionals to ensure clients receive appropriate medical care and treatment.
Facilitate referrals to specialists, diagnostic services, and other healthcare resources as needed.
Advocate for clients' healthcare needs within the healthcare system and community resources.
Educate clients on preventive healthcare measures, medication management, and healthy lifestyle choices.
Connect clients with relevant resources to address social determinants impacting their health.
Maintain accurate and up-to-date records of client interactions, assessments, and care plans.
Compile and analyze data for reporting and program improvement purposes.
