Purpose The Nurse?Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Nurse home visitors report to the NFP Nurse Supervisor.

Essential Responsibilities and Duties:

• Provides home visits to pregnant women and their families eligible for the NFP Program. • Adheres to nursing process and the NFP evidence?based model of home visitation. • Carries a caseload of 25?30 clients.

Working Conditions • Must be able to work a flexible schedule including some evening and weekends • Able to complete duties under stress, deadlines, and while attending to multiple tasks simultaneously. • Prolonged computer related exposure, as well as sitting and standing at workstations for long periods of time. • Work assignments will involve travel throughout designated area to multiple locations, including government and private agencies, private residences and community partner agencies.

Skills • Written and Verbal Communication ? NHVs need to communicate effectively, openly and consistently with their families, colleagues and stakeholders. • Time Management ? NHVs who can flexibly schedule and reschedule a day’s activities based on multiple factors. • Listening ? interactions between NHVs and clients depend on the NHVs ability to listen attentively and accurately to the client’s current experience. • Ability to impose structure ? in between and during home visits, NHVs must be able to create structure in moments of confusion, disorganization or crisis.
