Purpose of Job
The purpose of this classification is to provide prenatal navigation, and basic care to patients as well as leading the health service clerks team in administrative duties. Participate in daily supervision, clinical direction, organization and planning.
Essential Functions
The Licensed Practical Nurse-Team Lead-Presumptive Eligibility is responsible for the following essential functions:
Provides daily supervision of assigned staff in designated clinic, maintains staff time and attendance, provides guidance and expertise and participates in conflict resolution and addressing concerns
Participates in selection, training, handling employee concerns, counseling, and contributing to evaluations
Will learn health service clerk role to provide leadership support and coverage
Interpret and enforce agency policy and procedure with support of Clinical Nurse Program Manager
Perform point of care testing, compartment swab, phlebotomy and specimen packing and transport
Evaluate clients for program eligibility for the Tennessee Breast and Cervical Screening Program (TBCSP), Family Planning, Presumptive Eligibility, and other services (including but not limited to WIC, Quitline, breastfeeding, etc.) and referred or enrolled accordingly
Provides prenatal navigation and counseling
Perform a minimum of three follow-up prenatal navigations phone calls, documenting barriers, resolution of barriers, and any other assistance provided in the client’s medical records, track and follow up with clients to monitor compliance with referrals, screenings, diagnostics, and/or treatment
Collect and submit required data pertaining to specific programs or care pathways
Collect health information and accurately document in electronic health record system
Prepare patients for medical treatments under direct supervision of clinical nurse program manager, nurses and nurse practitioners
Take and record vital signs as necessary collect health information and accurately document in electronic health record system
Administer vaccinations and other injections as needed
Issue medications prescribed by provider
Observe patients and report adverse reactions to medical personnel in charge
Perform phlebotomy
Communicate moderately complex information to coworkers and patients
Perform other duties as assigned by the clinical manager
Maintains knowledge of legal limitations while performing job functions.
Participates in continuing education programs, attends in-service training annually; attends mandatory CPR class yearly; attends regular staff meetings
Perform duties in a variety of setting including Women’s Health, Mobile Medical Unit and correctional facility
